| The UnitedHealthcare "Choice HMO" plan is one of two medical plans offered at Miami Children's Hospital. The HMO (Health Maintenance Organization) plan has a convenient "Open Access" feature that allows you to see any provider within the HMO network without having to consult your Primary Care Physician (PCP). The HMO network is vast and offers a full range of services, including preventive care services and wellness services, for a modest co-payment with no deductible or claim forms to file. Preventative care services are provided at no cost ($0 co-pay) to the employee. In general, you must use the physicians, hospitals and laboratories within the network to receive coverage. |

| The UnitedHealthcare Preferred Provider Organization (PPO) plan is one of two medical plans offered at Miami Children's Hospital. The UHC PPO plan allows you to choose an in-network provider or to go outside the network, if you choose. Out-of-network services are subject to a higher deductible and out-of-pocket maximum, and you would be subject to coinsurance. In contrast, visits to in-network primary care physicians and in-network specialists are subject to modest co-payments. This plan offers you the flexibility to choose your preferred provider, whether within or outside the provider network, and still receive coverage. Preventative care services are provided at no cost ($0 co-pay) to the employee. |

| The Dental Health Maintenance Organization (DHMO) plan from Delta Dental is one of two dental plans offered at Miami Children's Hospital to give you coverage for preventive, diagnostic and restorative dental care, as well as orthodontic services. This plan provides complete coverage for regular preventive services within the Delta Dental provider network. Regular dental care is essential to maintain good overall health and this plan offers an affordable option for receiving the services you need to maintain excellent dental health. Office visits are subject to a modest co-payment. Under this plan, oral exams, cleanings (once every 6 months), and x-rays do not require a co-payment. In-network restorative services are covered for a fixed co-payment which varies according to the type of service. In order to receive coverage, you are required to use an in-network primary dentist and to obtain referrals for specialists. |

| Delta Dental's Preferred Provider Organization (PPO) plan is one of two dental plans offered at MCH. This plan allows you to use providers who are outside the Delta Dental network for preventive, diagnostic and restorative dental care, as well as orthodontia. You do not have to select a primary dentist or have a referral to see a specialist. Service charges are calculated in accordance with the PPO fee schedule. All services are subject to a yearly deductible and out-of-pocket maximum. As part of the PPO plan, you can always lower your costs by using in-network dentists who will only charge you up to the negotiated rate. |

| MCH offers vision and eye health coverage through a plan administered by EyeMed Vision Care. Routine eye exams are important not only to check your vision but also to screen for more serious conditions such as glaucoma, cataracts, diabetes, etc. For children, regular eye exams can detect problems that may impact learning and development. The EyeMed vision plan offered at MCH offers very affordable vision care services, especially when you use a provider in the EyeMed Vision Care "Select" network. Coverage is available for out-of-network services but at a higher cost. Through the plan, you receive coverage for vision exams and eyeglass lenses or contact lenses. Frames are covered every 24 months. There is no co-payment for annual vision exams. |

| MCH fully funds a group life plan for you with a benefit amount equal to two times your annual earnings rounded to the next highest $1,000 (to a maximum of $300,000). Life insurance policies are considered portable plans and, as such, if your employment with MCH ends, you may be eligible to convert this plan to an individual policy. |

| Supplemental AD&D (Accidental Death & Dismemberment) may provide coverage equal to $50,000, $100,000, $200,000 or $300,000 (as elected annually) for a modest premium. AD&D policies are considered portable plans and, as such, if your employment with MCH ends, you may be eligible to convert this policy to an individual policy. |

| MCH offers supplemental life insurance through The Hartford Group. Under this plan, you can choose additional life insurance coverage for yourself equal to 1, 2 or 3 times your annual earnings up to a specified maximum. |

| Supplemental life insurance may provide coverage for your spouse and/or your dependent child(ren). Coverage for your spouse can be either $10,000 or $25,000, and coverage for your child(ren) can range from $5,000 to $10,000, depending on the age of the child. |

| MCH fully funds both a short-term and a long-term disability plans on your behalf. The short-term disability (STD) plan pays 60% of your earnings up to a specified maximum if you cannot work because of a disabling illness or injury. The long-term disability (LTD) plan begins when the short-term disability period ends (or after 90 days, whichever is greater). LTD also provides 60% of your monthly earnings up to a specified maximum for a period of up to 180 days. Both insurance plans, which are fully funded by MCH, give you income protection in the event that you become disabled and are unable to work. |

| Employees are automatically enrolled in the hospital's Pension Plan after having met the eligibility requirement (1 year of service with 1,000 hours of service during the plan year). Under this plan, MCH contributes 3% of your eligible annual earnings to your pension account. To be fully vested and therefore eligible for distribution of your pension account balance, you must have completed three or more years of service (note: a year of service is equal to a calendar year in which you complete 1,000 or more years of service). |

| MCH's 403(b) plan allows you to save for retirement while reducing your taxable income by contributing to the plan on a pre-tax basis. The plan includes a wide variety of investment options to grow your account over the long-term. See the section on tax savings for details on how your 403(b) contributions helped to reduce your annual income tax obligations. |

| MCH will match your contribution for every dollar you contribute to your 403(b) Elective Deferral up to 3% of your annual salary (IRS limits apply). Once you become eligible, after 1 year of service with 1,000 hours of service during that year, MCH will begin contributing the employer match based on your elective deferral up a maximum of 3%. After eligibility period, 1,000 hours of service are not required to obtain the employer match. |

| Flexible Spending Accounts (FSA) allow you to use pre-tax dollars to pay for eligible expenses not covered (or not fully covered) by your medical, dental, prescription drug and vision plans. By paying for these expenses, including deductibles, coinsurance, co-payments, and other eligible expenses with pre-tax dollars you save money by reducing your taxable income. | Dependent Care FSA lets you pay for eligible expenses, such as daycare costs for a dependent child (under age 13) or an adult dependent, on a tax-free basis. By paying for these expenses with pre-tax dollars you reduce your taxable income, thereby saving money on your federal tax return. | MCH provides eligible staff with a generous paid time off (PTO) policy to enhance work/life balance. Under the policy, eligible employees can begin accruing PTO hours as of their date of hire. PTO hours can be used for any instance including vacation, personal time, sick time, holidays, etc. Unused PTO can carry over from one year to the next, up to a maximum number of hours. MCH also allows staff to cash out unused PTO hours (up to 80 hours). The PTO cash out option needs to be elected in one year in advance during the Open Enrollment process. |

| MCH offers an Employee Assistance Program (EAP) through Magellan, which provides a confidential and easily accessible professional counseling service to MCH employees whose personal problems are affecting their ability to function effectively at work or home. You and your family are eligible for this free service. The MCH EAP program provides free confidential professional counseling service to address the following types of issues: work stress; family/parenting issues; marital or relationship problems; anxiety or depression; grief or bereavement; anger management; alcohol or drug dependencies. |
